1 millionth blood stem cell transplant marks major medical milestone

The Worldwide Network for Blood and Marrow Transplantation (WBMT) announced the landmark achievement today. The WBMT—a nonprofit scientific organization whose mission is promoting excellence in stem cell transplantation, stem cell donation and cellular therapy—said the 1 millionth transplant occurred in late December 2012. The finding is based on data collected by WBMT international member organizations involved in blood stem cell transplantation, which were analyzed and verified by the WBMT. "One million transplants is a milestone that may surprise many people, because blood stem cell transplants were viewed as a rare procedure until the last decade or so," said Dietger Niederwieser, M.D., president of the WBMT and professor of medicine in the division of hematology and medical oncology at the University Hospital of Leipzig, Germany. "But important discoveries—and the vital cooperation of many scientists and physicians around the world—have dramatically improved outcomes for patients who undergo stem cell transplantation."

(WASHINGTON)- Transplants of blood-forming stem cells from umbilical cord blood may be an effective alternative to transplants of matched donor bone marrow stem cells to treat children with a rare, debilitating disease known as Hurler’s syndrome (HS), according to results of a study published online today in Blood, the Journal of the American Society of Hematology (ASH).

ESH Eurocord-Ed Eurocord World Cord Blood Congress IV and Innovative Therapies for Sickle Cell Disease

The upcoming World Cord Blood Congress will cover a wide range of topics including cord blood transplant in adults and children, the role of HLA in cord blood transplant, stem cells, cord blood banking and regulatory issues. An international panel of distinguished scientitists and clinicians will participate in this major international forum over four days of interactive conferencing. The scientific programme will comprise a series of plenary presentations and debates. Short oral communications selected from submitted abstracts will enable registered participants to present their own data.
The World Cord Blood Congress is open to all professionals working in fields related to cord blood biology and clinical applications from both public and private sectors, including physicians, research scientists, technicians, data analysts and nurses. The congress will also be of interest to healthcare policy decision makers.

ESH – EBMT Training Course on HAEMOPOIETIC STEM CELL TRANSPLANTATION
Syracuse in Sicily will host the next ESH-EBMT Training Course on Haemopoietic Stem Cell Transplantation will be held from April 25th to 28th, 2013. The meeting will be chaired by Eliane Gluckman (Paris), Tamas Masszi (Budapest), Jane Apperley (London) and Enric Carreras (Barcelona). The local host this year is Dr Giuseppe Milone (Catania).
Every year, the ESH-EBMT Training Course provides a comprehensive training in all aspects of autologous and allogeneic stem cell transplantation and comprises a mixture of formal lectures, case presentations by attendees and faculty, meet-the-expert breakfasts and dinner table discussions. For each topic an explanation of the basic principles is provided, followed by development of the subject through to the most recent advances in understanding and management. The objective of the course is to give opportunity to the attendees to have close contact and lively interaction with the teachers and the group in a nice setting. All slides will be available after the course on ESH and EBMT websites. The speakers are chosen not only for their expertise in their subject but also for their teaching skills and willingness to discuss the field of transplant in both formal and informal settings. The course is suitable for physicians at all stages of their career and is particularly useful for those in training, for those who wish to develop their current practice to a more challenging type of transplant and also as an update for established transplanters

To maximize the
opportunity to interact with our colleagues in the Asia Pacific, the
workshop at the ISCT Annual Meeting will include two tracks – one for
cellular therapy and one for cord blood banking. The agenda addresses
current events and future challenges, including experience and advice
from FACT representatives and accredited organizations. Cellular
therapy programs will learn about laboratory controls, ISBT 128
implementation plans, exceptional release, and accreditation of
processing hematopoietic-derived products used for immunotherapy or
regenerative medicine. Cord blood presentations include validation
studies, cord blood unit specifications, cord blood unit
characteristics post-processing and post-thaw, and the impacts new
research is having on cord blood banks. Separate registration from the

ESH-EBMT Handbook on Haematopoietic Stem Cell Transplantation 6th Edition

The new fully revised 6th edition of the ESH-EBMT Handbook will be available for the first time on-site at the ESH Booth (Number 417) at the upcoming EBMT Annual Meeting in Geneva, Switzerland April 1-4, 2012.
The editors, Jane Apperley, Enric Carreras, Eliane Gluckman, Tamas Masszi and over thirty-five international contributors have completely rewritten all the chapters and updated the list of references to reflect changes in practice. Comments and suggestions from readers of the 5th edition were taken into consideration by the authors and new sections have been added. These include a chapter on JACIE and one on indications for transplant and post-transplant immunotherapy.
The remaining chapters cover topics related to haematopoietic stem cell transplantation from the history of HSCTs, their biological properties to their clinical applications in malignant and non-malignant disease. Readers will also find chapters on statistics and methodology for conducting clinical trials in Europe.
As with the 5th Edition, multiple-choice questions (MCQs) are included at the end of each chapter to allow the reader to test their knowledge. The responses to the MCQs will be available online via the Eurocord-Ed website.
It is the sincere hope of all contributors that readers will find this edition even more useful and relevant than the previous one and that it will contribute to a better understanding of HSCTs and their use in treatment of patients.
Cord Blood Related Sessions at the Annual EBMT Meeting in Geneva, Switzerland

The European Group for Blood and Marrow Transplantation will hold its annual meeting in Geneva, Switzerland from April 1-4, 2012. Several sessions and a multitude of abstracts, posters and oral abstract presentations will cover key and emerging issues in Cord Blood transplant, technology and clinical applications.
These will include a workshop on choosing cord blood units chaired by Annalisa Ruggeri (Paris, FR) and Vincent Kindler (Geneva, CH) in which Vanderson Rocha (Oxford, UK) and Arnon Nagler (Tel Hashomer, Il) will present respectively choosing either single, double CB unit or an intramedullary application and the how, when and for whom of cord blood transplant.
Experts Thomas Bart (Berne, CH) and Gesine Kögler (Düsseldorf, DE) will debate the affordability of cord blood banks in a “Controversy” session chaired by the Eurocord-Ed partner who leads the “Quality Assurance” section, Sergio Querol (Barcelona, ES).
The World Marrow Donor Association and EBMT will present a joint session on the challenges related to the exchange and use of HSCs for transplantation. Eurocord-Ed and WMDA president, Eliane Gluckman (Paris, FR) and EMBT president, Alejandro Madrigal (London, UK) will be chair the session. Speakers will present new data on the impact of HSCs and stem cell source on donors and recipients. Members of several Japanese transplant societies will present on the consequences of earthquakes on unrelated transplants in their country.
In addition to these cutting edge sessions, abstracts will be presented on key topics such as HLA disparity on transplant outcomes and the effect of thymosine alfa I administration on immune reconstitution.

conducting an evaluation of their processes with the view to harmonise their

Professor Eliane Gluckman is one of the co-chairs of the upcoming ESH Updates in Clinical Hematology conference to be held in Paris on February 9-10, 2012 in Paris.
This conference is part of a new series features: a 1,5 day update on patient
management from world class experts, a clinically and practically
oriented program that reflects most recent state-of-the-art as well as
innovation, in a mixture of different hematologic disorders, – in-depth
presentations followed by interactive discussions, including clinical
cases and controversies, and a handout comprising lecture summaries,
slides and recent, selected references.

Eurocord-Ed Policy Document on Cord Blood Banking in Europe

Since the first cord blood transplantation was performed in
Europe in 1988, a large number of cord blood banks have been established in
order to facilitate unrelated cord blood transplantation. Over the years the
number of units collected in Europe has increased to approximately 190 000 in
2011. Cord blood transplantation
is a rapidly growing field with an increasing number of indications worldwide.
The results reported by Eurocord reflect a strikingly rapid increase in
activity. For example, the Asian Pacific Blood and Marrow
Transplantation Group reported 937 cord blood transplants in 2008 alone. In the
USA, more than 40% of paediatric unrelated transplants are provided by cord
blood.

Highlights from the 2011 World Cord Blood Congress

The ESH, Eurocord, Eurocord-Ed EBMT World Cord Blood Congress held in Rome, Italy from October 27-29, 2011 was a tremendous success thanks to the beautiful surroundings offered by the historic site in the city centre, the quality of the scientific programme, and the lively interaction with attendees.
The congress took place in the centre of Rome on the Piazza Montecitorio adjacent to the Italian parliament building. Over 320 participants from fifty-two countries and from as far away as Canada, Argentina China and Australia attended the event.
The 2011 meeting focussed on Cord Blood Transplant, immunological aspects, and clinical results. All sessions were divided into three key sections: the current State-of-the-art, expert short communications, and panel discussions.
According to attendee feedback, the possibility to discuss openly with internationally renowned specialists in both scientific and social settings was particularly appreciated The local organizers: William Arcese, Franco Locatelli, and Paolo Rebulla all deserve special thanks as do the Staff of the European School of Haematology. Allogeneic cord blood transplants are being offered to pediatric patients since 1995 in Turkey. The number of sibling-family CBTs have reached the number of twelve. The number of pediatric transplants for which the stem cell source is both cord blood and bone marrow is more: 51. Recently experience with both pediatric (n:49) and adult (n:15) unrelated cord blood transplants are amounting steadily. These number constitute approx. 3 % of all the allogeneic transplants but the trend for increase both in number and survival rates are promising. The main source of unrelated CBTs are from CBBs in Spain , NMDP, Italy and Germany. Until recently there was only autologous CB banking in Turkey limited to three private and a university CBB. In 2009 Ankara University, where the first CB storage (1994) and CBT ( 1995) was initiated was able to receive a grant that equaled to approx. 3 million euros which was spent on building a CBB with a GMP facility; renovation of the EFI accredited HLA typing laboratories, the Apheresis Unit, TRAN (Ankara University Donor Registry) and the Donor Center. The Ankara University CBB and Donor Registry are registered under BMDW for many years. Following the completion of the renovations and registration approval by the Ministry of Health, TRAN has started to recruit adult donors and cords blood donations. The grant will enable 9 000 donor (600 CB units included) costs of typing and storage. This will increase the donor and CB capacity from 4000 to 11000. On September 23 the CBB organized a opening ceremony and a scientific symposium. ESH, Eurocord and WMDA president Prof E.Gluckman and EBMT president and Anthony Nolan Director Prof A.Madrigal were the guests of honor. To an audience of 150 , the CBT experience of Turkish centers were presented which were followed by two state of art lectures by Gluckman and Madrigal. The following day a round table case discussions on best donor-CB selection was performed in the Registry site with attendees of TC coordinators. Overall the ceremony and the symposium was an important achievement in the history of transplantation in Turkey thanks to the contributions of a distinguished faculty.

Welcome to the Eurocord-Ed Website, on this page you will find information on how to use the website to find specific information relevant to your area of interest.
The website is designed to be a multilingual educational tool for vocational training destined for cord blood technicians, scientists, and clinicians including haematologists, immunologists, obstetricians, gynecologists, midwives, as well future parents and healthcare decision makers.
The information available on Eurocord-Ed comes from expert sources and has been peer-reviewed by the members of this academic Consortium.
Eurocord-Ed promotes a Blended Learning approach through provision of a broad range of relevant online learning formats and documents, and the promotion of Eurocord-Ed and other live conferences. Documents available on the Eurocord-Ed website includes texts, slide presentations, audio-visual webcasts, clinical case studies, expert forums, and links to relevant websites.
As the field is rapidly evolving, it is important to visit the site regularly to be kept abreast of the latest information. New content is added to the site on a weekly basis.
Eurocord-Ed is a multilingual website. The ‘Regulation, Ethics, Quality’ section of the website offers content in all EU languages. All other printed information on the website can be translated by clicking in the ‘Select your Language’ function on the top right hand side of the homepage (English, French, Spanish, Italian, German). And you can translate any printed information by copying it into the Reverso tool that is freely available on the website to all Eurocord-Ed members.
The Eurocord-Ed Consortium has enriched the Reverso translation possibilities through the development of an English-Italian cord blood terminology dictionary which will allow you to reduce time and costs related to professional translation by approximately 50%. This Eurocord-Ed product will be extended to other languages in future.

The European Commission is planning to put forward, in 2012, a legislative proposal to revise the Clinical Trials Directive 2001/20/EC.
This concept paper

- a 'preliminary appraisal' of which option appears to be the most suitable one to address some of the key concerns of the Clinical Trials Directive, on the basis of the current state of the impact assessment; and

European Medicines Agency releases new guidance on medicines based on SCs

The European Medicines Agency has released new guidance on medicines based on stem cells. The guidance covers the different types of stem cells used in medicines, and the considerations that companies need to take into account when developing medicines including stem cells or developed from stem cells.
The reflection paper on stem cell-based medicinal products (available here) stresses the fact that companies developing medicines including stem cells need to pay close attention to the way the medicines are manufactured, to make sure that the final medicine is as consistent and reproducible as possible.
Pre-clinical and clinical testing also needs to take account of the cells' properties, ensuring that the possible risks of tumour development and rejection by the body are studied adequately and balanced against their benefits for patients.
Stem cells are naturally occurring cells in the body that have the ability to divide and produce a range of different cell types. The use of stem cells as medicines is a promising area of research as they may be able to help the body to regenerate damaged or lost tissue in diseases like Parkinsons disease, multiple sclerosis, heart disease, liver disease, spinal cord damage or cancer.
The reflection paper was adopted by the Agency's Committee for Advanced Therapies (available here ) (CAT) on 14 January 2011. This followed a public consultation between March and June 2010 (see the overview of comments here ) and discussion at a public workshop in May 2010 here involving 230 participants from academia, industry and regulatory authorities. Researchers at Monash Medical Centre in Melbourne, Australia, are awaiting approval for a study that would use umbilical-cord blood to treat 20 children with cerebral palsy. The researchers want to evaluate

The World Health Organisation presents their guidelines for transplantation of human cells, tissues and organs. According to the WHO "In 50 years, transplantation has become a successful worldwide practice. However, there are large differences between countries in access to suitable transplantation and in the level of safety, quality, efficacy of donation and transplantation of human cells, tissues and organs. The ethical aspects of transplantation are at the forefront. In particular, the unmet patients needs and the shortage of transplants lead to the temptation of trafficking in human body components for transplantation." Eurocord is an international registry of cord blood transplants established in Paris, in 1995 under the direction of Dr Vanderson Rocha and Professor Eliane Gluckman. It has currently collected and validated more than 6000 cases of cord blood transplants throughout the world with approximately 70% provided by EBMT transplant centers.
EUROCORD collaborates closely with cord blood banks and EBMT centres, collecting and validating clinical data of patients transplanted with Netcord cord blood units, or receiving a cord blood transplant in an EBMT centre. This collaboration is important in order to evaluate the quality of the cord blood unit and to help cord blood banks to follow national and international system accreditation.
EUROCORD also collaborate with EBMT working parties in observational studies. In 2009, the EBMT Registry has finalized a data agreement with the EUROCORD Registry, to improve data collection in Promise. The agreement facilitates the data flow, enabling EBMT centres to submit their cord blood data once, instead of to both organizations.
In December 2009, in order to collect data of American patients receiving a European cord blood graft and vice-versa, an agreement between Eurocord-EBMT and CIBMTR was established to share clinical data outcome through the implementation of AGNIS project. AGNIS is "A Growable Network Information System", which has the goal to transmit haematopoietic stem cell transplant (HCT) data between different data bases. This project was approved and financed by the American government and it is in currently development between Eurocord and CIBMTR.
In 2009, Eurocord registry has published many articles (see below) in collaboration with EBMT working parties and National Societies. Two abstracts were accepted in the EBMT plenary session in Goteborg 2009. Prospective trials in collaboration with The French Society of HSCT (SFGM-TC) have started. Many other observational studies are on going. The main task of the Eurocord registry in 2010 is to finalize the AGNIS project and to continue and establish collaborations with cord blood banks and EBMT working parties. Many observational studies are on going and we hope to finalize most of them.

Umbilical cord blood is now considered a drug by the FDA.
The new Guidance for Industry: Minimally Manipulated, Unrelated Allogeneic Placental/Umbilical Cord Blood Intended for Hematopoietic Reconstitution for Specific Indications requires cord blood banks to measure identity, potency (strength) and quality of cord blood prior to its intended use. The parameters recommended by FDA/CBER are often at odds with, and in many cases, apparently contradictory to, the regulations. The Guidance allows for an "alternative approach" and other comparable and equivalent assays to be performed, providing the assays have been validated.

The Eurocet Project

Article 10, paragraph 2 of the European Directive 2004/23/EC requires the Competent Authority to establish and maintain a publicly accessible register of tissue establishments specifying the activities for which they have been authorised. In addition, paragraph 3 states "Member States and the Commission shall establish a network linking the national tissue establishment registers".
The Eurocet Project, which was funded by the European Commission, has established The Eurocet Registry which will include the name and address of the Tissue Establishment, and the type of tissue and / or cell to which the authorisation pertains. The Ospedale Maggiore hospital in Milan was originally founded in 1456. The "Policlinico" (General Hospital), Mangiagalli and Regina Elena Ospedale Maggiore Foundation was officially opened in 2005. The mission of exploiting the potentials of cellular therapy and of performing biobanking research was given to a unit named the Center of Transfusion Medicine, Cellular Therapy and Cryobiology (CTMC).
Currently, the CTMC includes five sections: one of which includes the Milano Cord Blood Bank, with an inventory of 6627 allogeneic cord blood units, 350 of which have been transplanted in 21 countries (39 units in 2007). Published March 6, 2009

An ATMP is a medicinal product as defined in Directive 2001/83/EC and is either:
- a gene therapy medicinal product as defined in Part IV of Annex I to Directive 2001/83/EC;
- a somatic cell therapy medicinal product as defined in Part IV of Annex I to Directive 2001/83/EC; or
- a tissue engineered product as defined in Article 2 1 (b) of the ATMP Regulation (1394/2007/EC).

Since the first human cord blood transplant, performed twenty years ago, cord blood banks have been established worldwide for collection and cryopreservation of cord blood for allogeneic hematopoietic stem cell transplant. A global network of cord blood banks and transplant centers has been established for a common inventory and study of clinical outcomes.
